Asthma Diary (Peak Flow Record)

If you're asthmatic, it's a good idea to monitor your peak flow twice a day and write down your readings. That way, you can tell if your asthma is under control or if it's starting to worsen -- even if you don't have obvious symptoms.

If you have trouble identifying your triggers, you may be able to find a pattern by regularly noting down what you were doing when you started having symptoms.

Print out a new page of this diary each week, and fill in rows of the table every day.
